What Is Carebridge?

Carebridge provides compassionate, in-home, and community-based support for children and youth with mental health, medical, and developmental needs.

We bridge a gap between families and essential services – helping children thrive in their daily lives.

HCBS Services

Crisis Respite

Temporary, short-term care for children experiencing a crisis that makes it unsafe or difficult for them to stay at home. We provide a safe environment and professional support to stabilize the child and prevent hospitalization or placement in a more restrictive setting.

Planned Respite

Scheduled, short-term care that gives caregivers a break while ensuring the child is in a safe and structured environment. It allows parents or guardians time to rest, work, or handle other responsibilities while their child is engaged in supervised activities.

Prevocational Services

Helps older children and teens develop essential work-related skills like communication, problem-solving, and time management. These services prepare them for future employment by building confidence and teaching workplace expectations.

Caregiver / Family Advocacy And Support Services

Provides guidance, education, and emotional support to parents and caregivers of children with special needs. It helps families navigate available services, advocate for their child’s needs, and connect with useful resources.

CFTSS Services

Other Licensed Practitioner
(OLP)

  • Therapy provided by a licensed therapist in the child’s home, school, or chosen community setting.
  • Can include collateral sessions with parents, school staff, and other important figures in the child’s life.

Community Psychiatric Support And Treatment (CPST)

  • Support provided to both the child and their family to improve overall functioning.
  • CPST reinforces goals set by a therapist or social worker by helping families develop problem-solving strategies, strengthen communication, and create a supportive home environment.
  • Sessions may include guidance for caregivers, emotional regulation techniques, and family-based interventions to enhance stability and well-being.

Psychosocial Rehabilitation
(PSR)

  • Focuses on helping children and teens reintegrate into daily life.
  • Provides mentorship and structured support to address challenges such as school attendance, social skills, and independence.

Who Qualifies For CareBridge Services?

Carebridge is available to children and youth who:

  • Provides in-home care for children with serious medical conditions, reducing the need for hospital visits
  • Supports kids undergoing cancer treatments like chemotherapy or bone marrow transplants
  • Offers care for children with heart conditions, including post-surgery recovery and congenital heart defects
  • Gives extra support to children who frequently visit the emergency room
  • Includes mental health services for children with severe emotional or behavioral challenges
  • Helps keep children safe, supported, and cared for within their communities
